At your first appointment, you will need to bring the following.

• Co-pay, if applicable

YOUR EVALUATION 

Your therapist will ask you some questions about your current condition and administer various tests. Based on the therapist's findings, a treatment plan will be designed for you. Your appointment will last about 45 minutes.

CLOTHING 

Please wear loose and comfortable clothing. 

           Your Treatment Plan

 Your therapist and physican will determine how long and how often you will come to physical therapy. You will know what your treatment plan is before you leave your first visit. Your therapist will include you in every aspect of your plan of care.

You will make all of your appointments with the front desk coordinator before you leave.
